DO YOU TAKE INSURANCE?

We are out of network with all insurance companies, but if you would like us to check on your out-of-network benefits, we will gladly do this for you and can still submit to your insurance or provide you with a superbill if that is your preference.

If you don't have insurance, then ask about our self pay rate.

WHY ARE YOU OUT OF NETWORK WITH INSURANCE?

Being out-of-network with insurance allows us to provide the highest quality of one-on-one care for our patients. We therefore are not limited by insurance companies and that allows us to individualize your care to make sure you can return to whatever it is you enjoy doing most!

DO I NEED A DOCTOR'S REFERRAL?

You do not need a referral for chiropractic services. All practitioners are trained in orthopedic and neurological diagnostics and have the duty to refer appropriately. If there is a problem that we cannot treat, we will make sure you can find the right provider for your needs.

IT'S MY FIRST TIME COMING TO SEE YOU, DO I NEED A NEW PATIENT EVALUATION?

If it is your first time being seen for chiropractic care, then yes, a new patient examination is required. This allows us to collect the necessary patient information for our knowledge and allows Dr. Minen to complete a thorough examination and develop a comprehensive treatment plan.
If you have been to our clinic before and have a new injury, we recommended booking a 60 minute follow-up so that a re-evaluation can be performed of the new injury. If it is your first time to the clinic, please fill out our intake forms at your leisure prior to your session.
